Questionnaires and intervention materials were presented to NC Mutual policyholders by sales agents, who received training through printed and video materials.

- Agent training video for administering the Smoking and Health Survey
Phase I of the study consisted of a survey of policy holders to assess cigarette smoking prevalence (current, ex-, and never smokers) as well as smoking history and characteristics for current smokers. Results were published in the American Journal of Public Health.
- Agent training video for presenting the Quit for Life quit-smoking intervention
Sales agents in 16 randomly selected NC Mutual's sales districts provided the Quit for Life quit-smoking intervention to policyholders who had indicated their desire to quit smoking. Another 16 districts received the quitting intervention at the end of the 12 month study. This video was used to train agents to present the intervention packet and to conduct a follow-up visit.
