Please evaluate the other members of the class to let us and them know how helpful their participation was to you.

As indicated in the syllabus, to recognize your work in providing an evaluation of your peers, up to 2 evaluation points will be given for submitting the peer evaluation form (ratings on all members of the class = 1 point, ratings plus non-trivial comments for all peers = 2 points)

By 4/30, please write out your ratings (see list below) and comments. When you are ready to submit the evaluations, please go to http://courses.sph.unc.edu/evals/?course=epid600. You will need to log in with your Onyen.

Note that the webform will require that you enter a rating for every dimension for every student (or choose "Not applicable") in order to submit the form.

The average scores and comments about each member of the class will be compiled and distributed to her/him. However, only item #6, “Overall evaluation”, will affect the final grade. You can recognize one or more class members with a rating above 100% - but only a minority of your ratings can be that high (ratings will be adjusted down if too many exceed 100%). There is no limit on 100% ratings.

The identity of the rater will be known only to the instructors.

(Rating options for questions 1-5)
1 = 1 - Not adequate
2 = 2 - Adequate
3 = 3 - Fully satisfactory
4 = 4 - Excellent
5 = 5 - Outstanding
N = N - No Rating

Questions (note: this form is used for EPID600. For EPID799, please interpret "group" to mean the class, and please use the rating for "#4 Facilitating" to evaluate any presentations or discussions led by this student):

#1 Participating in group activities and discussions

#2 Preparing for group discussions (reading - assigned, suggested, and/or other; writing out answers to the case study)

#3 Making good quality contributions to the group's work and learning

#4 Facilitating, organizing, and leading group activities and discussions

#5 Supporting the other group members by being attentive, polite, respectful, fair, encouraging, appreciative, and helpful

#6 **Overall evaluation of how much this group member has contributed to the group and to your learning
120 = 120% - Outstanding contributor
110 = 110% - Superior contributor
100 = 100% - Exemplary contributor
95 = 95% - Excellent contributor
90 = 90% - Very good contributor
85 = 85% - Good contributor
75 = 75% - Adequate contributor
50 = 50% - Inadequate contributor
25 = 25% - Minimal contributor
0 = 0% - Non-contributor
(Note: if you assign ratings above 100% to half or more of your group members, the ratings above 100% will be adjusted downward - e.g., 90%, 90%, 110%, 110%, 120% become 90%, 90%, 100%, 100%, 110% because the median rating was 110%.)
